Anyone seeking an immigration medical exam is welcome. Dr. Luz A Lopez is a family medicine Board-certified physician licensed in the State of Vermont. She has over 25 years of experience working with people of all ages and nationalities.
These must be done WITHIN 12 months prior to completing the form. There are no exceptions to this requirement.
Lab tests that are required for the medical form:
TB blood test known as IGRA (NOT skin test—the skin test known as PPD is no longer admissible) — ages 2+
Syphilis screening — ages 18-44 only
Gonorrhea screening — ages 18-25 only
The immunizations required for those over the age of 18 years are:
Td or Tdap within past ten (10) years
Two (2) doses of MMR (measles, mumps, rubella)
Two (2) doses of varicella (chicken pox)
Two-Three doses of Hepatitis B vaccine (dependent on which brand you received)
Completed Covid series (Johnson&Johnson-1 dose, Moderna or Pfizer 2 doses)
Flu vaccine from Oct. 1-April 1
Pneumonia vaccine if over age 65
Depending on what country and date of arrival in US, polio and Hepatitis A vaccines may also be required
It is common that patients will have received all required vaccinations. However, USCIS will only accept medical documentation of vaccinations.
If you do not have documentation, you will need to receive these vaccines OR you can receive a blood test proving immunity for measles, mumps, rubella, hepatitis B, and varicella. These tests can be completed at your initial appointment. If your blood test shows that you are not immune, we can then discuss how to receive the vaccines for the lowest cost possible.
The completed I-693 Form will be provided upon payment by cash or check at your follow-appointment.
Payment for vaccinations is directly to the pharmacy where administered.
I-693 Forms are typically completed within a week of the initial appointment, unless the patient requires additional medical care. Ultimately, a completed I-693 will be provided in a sealed envelope pick-up, in addition to copies of the I-693 and supporting documents (for personal records).
All children over the age of 2 must have a TB blood test.
Anyone under age 18 is required to have all the vaccines as recommended for their age group by the CDC guidelines.
Most children will already have most, if not all the required vaccines. However, because the required vaccines for children are so extensive, I prefer to review these as the initial step in the process. This way, if any further vaccinations will be required, the parents have adequate time to schedule appointments to have these completed.
